The specialized staff at Community Home Care work under physician orders to provide homebound patients across Southeast Georgia with skilled in-home care. The services provided by our staff allow patients to heal from the comfort of their home, while continuing to receive necessary medical treatments. Community Home Care employs professionally registered and licensed nurses, therapists, social workers, and certified nurse aides. Patients are referred to us for many reasons, some of which include: post hospitalization care, wound care, post surgical care, disease management, and home rehabilitation programs. Community Home Care is certified and licensed by the state of Georgia and is Joint Commission accredited. We accept most insurance including Medicare, Medicaid, MRP plans, Private and Contract Care

To qualify for services with Community Home Care, patients must be referred by a physician.

We are able to serve patients who reside in:

Appling, Brantley, Long, Pierce, Tattnall, and Wayne Counties
