What is the process?

Our office coordinates with physicians to provide in home care for patients who reside in our service area.


Patient's must be referred for services by a physician.

These referrals are sent directly to our office and received by our patient intake specialists who will then contact you.

During this time, our office will confirm insurance coverage to prevent any "surprise" out-of-pocket costs for the patient.

Shortly after enrollment, a nurse or therapist will make contact to schedule an admittance evaluation. This visit takes place within 48 hours unless otherwise approved by your provider.

The assessment will determine your needs such as frequency and specialty of care. The professional performing this initial visit will also discuss valuable information such as emergency preparedness, patient's right to privacy, and payment policies.

Patients are encouraged to take an active role in developing an appropriate plan of care that meets their specific needs.

Depending on individual patient needs, a patient may see any number of our specialized medical team.

Once your goals are met, you will be discharged from our service. We encourage you to fill out the patient satisfaction survey or leave us a review. This information will be used to measure for improvement and satisfaction.

Who pays for these services?

We accept most insurances including:

  • Medicare

  • Medicaid

  • MRP plans

  • Private insurance

  • Self pay

Who can receive services?

We provide services to those who are:

  • Referred by a physician

  • Meet homebound requirements

  • Have need of specialized skilled medical care

How can I get services?

Call our office and we can help with the referral process.

Talk to your physician. If your physician feels there is a need, he or she can send us a referral to begin services.
